Our Vision
The vision of Ultra Care Hospice, Inc. is to partner with our patients and caregivers, physicians, employees and volunteers to be the premier provider of Hospice Care:

- We will actively listen to our key constituents and be flexible and creative in meeting the needs of our community;
- We will set high standards in our management systems, producing necessary data to support efficient and effective organizational and patient outcome management;

What is Hospice Care?
Hospice focuses on caring rather than curing. It helps people who are terminally ill live as fully as possible for all the time that remains. A unique and individualized plan of care is compassionately provided by our hospice team at whatever location the patient calls home, including long-term care and Residential Care Facilities.
Where is Hospice Care Provided?
Who Can Refer a Patient?
Anyone can refer a friend or their loved one to a hospice program. The patient and family should feel free to discuss hospice care at any time with their physician, other health care professionals, clergy, or friends.
What is Needed to be Eligible for Hospice Care?
Patient must be determined to have a terminal illness, with a prognosis of 6 months or less to live, if the disease follows its normal course; has made the decision to receive "palliative care" (pain relief) instead of curative measures; and wants comfort measures.
